Making a Referral

Please complete the referral form and return it to [email protected] or if you have any questions or want to discuss your referral, do call us directly on 01733 558301.

How it works

Once we have reviewed your application form we will get in touch.  If it is appropriate, we will arrange two meetings:

• The first is an informal meeting to chat 
• At the second meeting our Empowerment Worker will discuss and complete a needs, risk and strengths assessment with you.

If we agree we can work together, then we discuss next steps for visiting the home and moving in.

Referral Form

Once you are in a Hope into Action home, we can help you to live independently, sort out your tenancy, help with benefits, work with other agencies, try to get you volunteering or in employment/training.
